Breathwork Session Waiver and Release Form

I, (purchaser), understand that participating in an online breathwork session with Leanna Bishop (whether online or in person) involves engaging in breath practices and potentially intense physical and emotional experiences.

1. Voluntary Participation:

I acknowledge that I am voluntarily participating in this breathwork session and that I am at least 18 years of age.

2. Understanding Risks:

I understand that breathwork can involve physical and emotional releases and changes, and that there can be inherent risks involved, especially for those with mental health and certain physical health conditions.

3. Medical and Mental Health Responsibility:

I confirm that I do not have any health conditions that would make breathwork inadvisable, including, but not limited to, recent surgery, cardiovascular issues, high or low blood pressure, history of seizures, or certain psychiatric conditions.

I agree to inform the facilitator of any physical or mental health issues or concerns before the session.

4. No Substitute for Professional Advice:

I understand that Leanna Bishop is not a licensed psychologist, healthcare professional, or medical doctor, and that breathwork is not a substitute for professional health care, mental health care, or therapy.

5. Confidentiality:

I acknowledge that discussions during sessions are confidential but understand that confidentiality will be broken if there is a threat of harm to myself or others.

6. Release of Liability:

I hereby release and discharge Leanna Bishop, her agents, representatives, and successors from any and all claims, liabilities, damages, actions, causes of action, costs, and expenses, whether at law or in equity, whether known or unknown, arising out of or in connection with any injury, illness, or damage that may result from my participation in the breathwork session.

7. Purchasing and Agreement to Terms and Conditions:

• I acknowledge that by purchasing a session, I confirm I have read, understand, and agree to the terms and conditions outlined on Leanna Bishop's website.

• I understand that it is my responsibility to be aware of and adhere to these terms throughout my engagement in services provided by Leanna Bishop.

8. Acknowledgment:

I have read and understood this waiver and release form.

I acknowledge that I have had the opportunity to ask questions regarding this waiver and the breathwork session.
